Coach – Girls Assistant Varsity Basketball

• Certificate Copy (Fundamentals of Coaching Education; Concussion; Sudden Cardiac Arrest and Heat Illness Prevention)
• CPR/First Aid Certification (Must have valid CPR and First Aid Certificate)
• Driver’s License Copy (must be valid)
• Letter of Introduction
• Letter(s) of Recommendation (Must submit two (2) letters dated within the past twelve (12) months.)
• Proof of HS Graduation (or equivalent)
• Resume
