Acute Care Associate Provider (PA/FNP)

As a Nurse Practitioner or Physician Assistant, you will provide same day access for One Community Health patients in our Clinic located in White Salmon. Being a community health center, our patients have access to numerous integrated supportive programs, including diabetes education, behavioral health, affordable pharmacy, perinatal, insurance navigators, wound care, and case management. We focus on providing personalized service in a collaborative and respectful environment.

We’re also looking for clinicians with an interest in leadership, and welcome meaningful participation in organizational decisions.

Patient care hours for this role are 11am-6pm, with 2 weekdays/week minimum, plus 2-4 weekend days/ month.

Minimum Qualifications

  • Graduation from an accredited allopathic or osteopathic medical school and current Board Certification in specialty field.
  • Current Washington NP or PA license and DEA licensure in good standing.
  • Minimum of three years of professional clinical experience in family practice or related field.
  • Experience working with diverse patient populations.

Preferred Qualifications

  • Bilingual English/Spanish
  • Experience with Epic EMR software
  • Current Washington NP or PA license for telehealth visits
