What You Will Do
- Serves as the initial contact of the coaching team, provides information on SFC services and requirements while guiding the caregiver and consumer through the referral process
- Educates prospective families on the benefits of Structured Family Caregiving (SFC) services
- Coordinates and completes all applicable assessments
- Coordinates consumer service activation and ensures that all required forms and standard procedures are followed for the activation process.
- Develops strong relationships with case management entities to drive collaboration
- May provide ongoing coaching to caregiver in conjunction with coaching team as needed to drive effective utilization of established best practices and protocols
- Develops, reviews and approves each member’s plan of care on an ongoing basis
- Provides clinical consultation, training and education to coaching team
- Support continuous Quality Improvement Programs while ensuring compliance to NCQA accreditation, state or contractual requirements
- Ensures continuous survey readiness and compliance with all regulatory and accreditation agency standards
- Performs other duties as assigned
What You Will Bring
- Must hold an active Registered Nurse (RN) License with a compact licensure
- Minimum two (2) years of recent experience in direct care of elders or adults with disabilities
- Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting
- Proficiency in common software and web-based applications (e.g. MS Office, case management systems)
- Additional state-specific qualifications may be required.
- Fluent in Spanish – reading and writing required
