Job Summary
We are seeking an experienced and motivated Nutrition Educator to join our team at a leading Diabetes Patient Monitoring (RPM) company. This role is critical to supporting our mission of empowering individuals to manage their diabetes effectively through personalized nutrition education and innovative technology. The ideal candidate will bring expertise in nutrition science, diabetes management, and remote patient engagement, along with strong managerial and organizational skills.
Key Responsibilities
Patient Education and Support
- Provide personalized nutrition counseling to patients enrolled in our diabetes RPM program
- Develop and deliver engaging virtual education sessions and workshops on diabetes management and nutrition best practices
- Collaborate with patients to create customized meal plans and lifestyle strategies tailored to individual needs and goals
- Program Development and Management
- Design and implement evidence-based nutrition programs specifically for diabetes care within the RPM framework
- Evaluate program effectiveness using patient outcomes and feedback, identifying areas for improvement
- Contribute to the integration of nutrition-related content into the RPM platform to enhance patient engagement and education
Data-Driven Care
- Analyze patient health and nutrition data collected via the RPM platform to provide actionable insights and interventions
- Work closely with the clinical and technical teams to optimize program delivery based on real-time patient metrics
Collaboration
- Mentor RPM staff, interns, or supporting the nutrition education component of the RPM program
- Partner with healthcare providers, endocrinologists, and other team members to deliver a cohesive and comprehensive care plan
- Serve as a subject matter expert on diabetes nutrition in internal and external meetings, presentations, and training sessions
Qualifications
Education and Certification
- Bachelor’s degree in Nutrition, Dietetics, or a related field
- Certification in diabetes education (e.g., CDCES) is preferred
Experience
- Minimum of 2 years of experience in diabetes care, nutrition education, or related fields
- Experience working with RPM platforms or telehealth services is a strong plus
- Previous leadership or program management experience is highly recommended
Skills
- In-depth knowledge of nutrition’s role in diabetes management, including carbohydrate counting, glycemic index, and meal planning
- Strong communication skills with the ability to engage, educate, and motivate patients in a remote environment
- Proficiency in using technology for patient engagement, including RPM tools and telehealth platforms
- Exceptional organizational and time-management skills to juggle multiple priorities effectively
