Scheduler – Home Health

Klarus Home Health / Jet Health, Inc is seeking a detail-oriented and organized Home Health Scheduler to join our team serving our home health patients and staff in our San Antonio Home Health office.

The primary function of this role is to schedule patient/client services and coordinate with staff, ensuring efficient and timely delivery of care.

Requirements

• Education: High School graduate.
• Experience: One year experience as a home health scheduler preferred.
• Skills: Exceptional communication skills. Experience with electronic medical records, HCHB preferred. Ability to multitask in a fast-paced environment.
• Transportation: Reliable transportation.

Environmental And Working Conditions

• Works in a routine office environment.
• Noise level may be moderately high.
• Ability to work a flexible schedule and extended hours.
• Some exposure to inclement weather.

Physical And Mental Effort

• Prolonged sitting and some standing required.
• Occasional need to lift, pull, carry, and push items weighing up to fifty pounds.
• Requires working under some stressful conditions to meet deadlines and patient/client needs, and to make quick decisions.
• Requires hand-eye coordination and manual dexterity.
• Required problem-solving skills.

Essential Functions

• Schedules and appropriately documents patient/client schedules in a timely manner.
• Participates in coordinating care with management and the patient/client interdisciplinary team.
• Provides effective communication to patients/clients, staff members, other healthcare professionals, and referral sources.
• Processes reports timely to ensure compliance.
• Responds to Agency and patient/client needs in a professional and creative manner.
• Promotes the agency philosophy and mission by presenting a positive image to patients, families, physicians, and community agencies.
• Demonstrates commitment, professional growth, and competency.
• Provides effective communication to patients/families, team members, and other healthcare professionals while maintaining confidentiality.
• Performs other duties as assigned.
