Postion Qualification Requirements
Education: Graduate of an accredited nursing program and/or bachelor’s degree with relevant healthcare experience.
Experience: Requires (3) three years’ experience working in a community health setting and with refugee and immigrant patients. Experience coordinating clinic services. Team leader with crisis management experience; sensitive to cultural differences of clients and staff from diverse backgrounds; functional vehicle required; able to work flexible hours.

Preferred: Candidate is bilingual (any language)
Transportation Requirement: Must possess a valid driver’s license and automobile with insurance coverage as required by the State of Missouri.
License(s)/Certificate(s): If nurse, LPN/RN license; CPR
Essential Functions
FUNCTIONS OF THE JOB
- Acts as the point person (primary coordinator) between resettlement agency and Affinia Healthcare to schedule medical assessment appointments, request interpreters, and address transportation needs of newly arrived refugees.
- Collaborate with outside organizations and partners to provide coordinated medical services for newly arrived refugees/refugee families.
- Educates clinic staff and others regarding culturally sensitive issues related to refugees.
- Maintains accurate written and electronic records in accordance with funding and agency requirements.
- Prepares reports and ensures compliance of requirements with the United States Committee for Refugees and Immigrants (USCRI) as directed by program leadership.
- Attends refugee resettlement quarterly meetings.
- Accountable for ensuring safe, accessible quality care.
- Works with subject matter experts to ensure current and consistent practices are adopted and sustained.
- Facilitate communication among program staff regarding program specific procedures, changes, clarifications, and distribution of pertinent materials.
- Assists in the management of refugee patient care between visits.
- Assists Refugee Program Manager and refugee team with patient flow. Acts as patient advocate to ensure needs are met.
- Completes appropriate screeners and documentation in accordance with organizational processes and/or contract administrator.
- Following defined protocol independently; follow chain of command and seek provider advice appropriately.
- Set-up appointments, e-mail, etc.
- Performs other duties as assigned.
