About The Position

Job Title: Registered Nurse (RN) Job Specialty: Home Health Job Duration: 13 weeks Shift: Days, 8 hours Guaranteed Hours: 40 hours weekly Experience: 2+ years of Nursing experience, 6 months of Home Health experience, Travel experience License: Active Hawaii license Certifications: Homecare Home Base, OASIS, COVID and FLU vaccine (declinations not accepted) Must-Have: – Active and clear driver’s license – Car insurance – Bachelor’s degree preferred – Case Manager experience preferred Job Description: – Provide excellent patient care by collaborating, developing, implementing, revising, and reporting on the case management program. – Act as a liaison between the patient, family, healthcare teams, and healthcare benefactors to optimize outcomes. – Serve as a consultant to the healthcare team on specific patient items. – Lead the coordination of patient care with other disciplines within the care team. – Monitor the appropriateness and timeliness of care to ensure the interdisciplinary care plan aligns with the patient’s clinical course and needs. – Discuss with physicians the appropriateness of resource utilization, consultations, treatment plans, estimated length of stay, and discharge plans. – Focus on complex cases such as frequent emergency department utilizers, chronic pain and substance abuse patients, and homeless patients. – Collaborate with acute care case managers to ensure appropriate ongoing care. – Coordinate the transfer of patients, including necessary information for continuity of treatment and services. – Provide oversight and collect data for regulatory and accreditation compliance. – Manage frequent emergency department visitors by conducting focus study reviews of past and current admissions. – Requires own transportation and to book accommodations in central areas for necessary coverage. – Rotating weekends required (2-3 per month).
