About The Position
- Job Title:** Home Health Registered Nurse **Job Specialty:** Care Manager, Home Health RN **Job Duration:** 20 weeks **Shift:** Days: Monday-Friday with weekend rotation **Guaranteed Hours:** 36 hours per week **Experience:** Minimum 1 year of experience required **License:** Registered Nurse License **Certifications:** OASIS experience required **Must-Have:** Home health and case management experience **Job Description:** – Provide care management services as a Home Health Registered Nurse. – Demonstrate strong home health and case management skills. – Utilize expertise in the OASIS documentation system. – Adapt to various patient needs in a home health setting. – Participate in a weekend rotation as part of your weekly schedule. – Work collaboratively as part of an interdisciplinary team. – Manage patient care plans and adjust as necessary based on patient condition changes. – Engage in patient and family education for ongoing care and wellness. – Maintain thorough and timely clinical documentation. – Demonstrate flexibility and adaptability, with no requested time off for the duration of the contract. – Experience as a traveling nurse is strongly preferred.
