The opportunity
We’re looking for a strong backend engineer to help design and build the foundation of our platform. As one of the earliest engineers on the team, you’ll partner closely with the Head of Product & Engineering and the CEO to shape the core technical direction that future products will rely on.
This is a ground-up product and platform build, giving you major influence over architecture, ownership of meaningful product areas, and a chance to help establish the engineering culture as the company scales. The ideal person enjoys moving quickly, is product-minded, and feels comfortable with a high level of responsibility.
About the company
VITL is a Nashville-based healthtech company focused on simplifying e-prescribing infrastructure for cash-pay medical practices. Its open pharmacy marketplace connects clinics with verified 503A compounding pharmacies across the country, offering real-time price comparisons, ordering across multiple pharmacies, and a patient tracking experience in a single interface. The company was founded in 2024 and supports more than 700 clinics serving 175,000 patients in the United States. It is backed by Signalfire, a San Francisco venture capital firm with $3B in AUM, which led the Series A in late 2025.
What you’ll do
- Shape and implement greenfield backend architecture, including APIs, database structure, infrastructure, and data pipelines, with performance and long-term maintainability in mind.
- Take end-to-end ownership of features, from user and stakeholder conversations through specifications, strategy alignment, and production release.
- Build technology that supports healthcare workflows and contributes directly to better patient care.
- Work in a rapid shipping environment where the team iterates quickly and learns from live user feedback.
What you bring
- At least 5 years of experience shipping production systems using modern backend stacks.
- Hands-on work with cloud services such as AWS or GCP, databases such as Postgres, MySQL, DynamoDB, Spanner, or similar, API development, data pipelines such as Airflow or Dagster, and languages like Python, Golang, Java, or Kotlin.
- A history of strong ownership in startup-like environments, including building from scratch, leading projects, founding something, working on a side project, or contributing at an early-stage startup.
- Comfort operating in ambiguity, moving quickly, and solving practical problems with strong attention to detail.
- Excellent written and verbal communication skills, including the ability to explain technical decisions clearly and convincingly.
Preferred background
- Experience with backend-as-a-service tools such as Supabase, Firebase, Railway, Hasura, or similar platforms.
- Exposure to AI-assisted development tools such as Claude Code or Cursor.
- Knowledge of healthcare systems and compliance topics such as HIPAA, EPCS, PDMP, and EHR/EMR workflows.
- Frontend familiarity with React, TypeScript, Next.js, or comparable technologies.
Compensation
The base pay for this role will be competitive and will depend on the candidate’s skills, background, and location. Total compensation may also include performance-related incentives, benefits, and company equity/options.

This position may involve access to PHI and PII. The person in the role will be expected to follow HIPAA requirements, internal privacy policies, and least-privilege access practices.
