About the Role
Apex Harmony Lodge is seeking a Case Manager who can coordinate people, processes, and support systems for residents living with dementia. The role goes beyond administrative case handling and focuses on delivering coordinated, compassionate, and person-centred care while also supporting families throughout important transitions.
This position is suited to someone who is comfortable working across different professional and family settings, can handle complex situations calmly, and is motivated by helping seniors maintain dignity and quality of life.

Key Responsibilities
- Oversee care plans so residents receive complete, person-centred support.
- Work closely with nurses, therapists, doctors, and other care professionals to coordinate quality service delivery.
- Develop trusted relationships with residents’ families and act as a main contact point.
- Support new admissions, care handovers, and discharge arrangements.
- Track resident progress and help arrange care reviews when needed.
- Contribute to audits, service reporting, and quality improvement work related to social care.
- Keep records accurate and aligned with applicable standards and compliance requirements.
- Take part in multidisciplinary discussions and contribute to care planning decisions.
- Represent residents’ emotional, social, and psychosocial needs and interests.
What We Are Looking For
- A Degree or Diploma in Social Work, Psychology, or a similar field.
- Prior exposure to dementia care, case management, healthcare, or community care will be an added advantage.
- Strong verbal and written communication skills, along with good people skills.
- Comfort working in a multidisciplinary team environment.
- Working knowledge of Microsoft Office tools, including Excel, Word, and PowerPoint.
- Authentic interest in supporting older adults and their families.
Role Purpose
This role is about more than managing cases. It is about helping residents live with dignity, giving families confidence and support, and ensuring no one has to navigate the dementia journey alone.
